What Is DTI MRI?
Diffusion tensor imaging, or DTI MRI, is an advanced MRI sequence that measures the movement of water molecules along the brain's white matter tracts, the bundles of nerve fibers that connect different regions of the brain. In healthy white matter, water tends to move in a consistent, directional pattern along these fiber tracts. When white matter fibers are stretched, sheared, or disrupted, as can happen with traumatic brain injury, that directional water movement can change in ways a standard MRI or CT scan isn't designed to detect.
Conventional MRI and CT remain an essential first step in any brain injury evaluation, built to identify structural findings such as bleeding or a visible lesion. But peer-reviewed research has shown that a meaningful number of patients with persistent post-concussion symptoms have normal conventional imaging despite measurable white matter differences on DTI. That gap, real symptoms with unremarkable standard scans, is precisely where DTI MRI is most useful.
Why DTI Matters When Standard Imaging Is Normal
For patients who continue to experience headaches, cognitive slowing, or dizziness weeks or months after a head injury, a normal MRI or CT can be confusing. It does not mean nothing happened; it means the imaging modality used wasn't designed to capture the kind of microstructural white matter change that diffusion-based imaging can measure.
